About: City Health Care Partnership CIC / Active Lifestyles Service

(as a service user),

I was referred to Hull exercise referral scheme (Active Lifestyles) when I asked my nurse at my yearly review of diabetes. I was referred to Liz Glenville at the KC stadium - I found it very informative. Helped me to change my diet, which I thought was healthy, her general advice was good. I go to exercise classes, and try to follow what I have learned. Thank you.
