(as a carer),

I am carer for my mum.

 

My mum has Alzhiemers, previously visited podiatrist, (she also has diabetes)

 

I requested at her doctor if she could have a home visit, I was advised to phone Podiatry at Newmains Health Centre, I have phoned three times in the past six weeks asking for a self referral form, I still havnt received one, not convenient for me to collect from health centre, as I work full time, and live thirty five miles away.
