Since I have been receiving care from the community rehabilitation team in the last two years I feel like a different person.
The staff there reviewed my medication and changed it, they encouraged me to get back to my hobbies which has been a great help as a distraction technique when I'm not feeling quite so good. They have also reviewed my mobility and given me aids to make me feel more secure and confident.
When my care switched to the community rehabilitation team I wasn't sure about it, there was inconsistent information about whether it was possible / permissible to call them, in fact they were happy to take calls at any time except during mealtimes and when they were handing out medication to their patients. I was very satisfied with this as it gave me confidence that their focus was on the patients at these times, making sure they were eating well and taking the medication prescribed although this did not apply to me as I was at home.
I was able to phone the team when I didn't feel so good, they would listen and be able to lift my spirit although they also exercised tough love when they needed to.
My family have noticed I am much better since I have been receive care from the Community rehabilitation team
