"my biopsy being labelled with other patients details"

I wrote to you previously regards problem with my medications on discharge. Patient Opinion published this of which I had a reply on the day of my procedure I had a biopsy of my bladder taken and waited several weeks for an answer to tell me the results.

I never heard anything so I made several enquires my gp my consultant’s secretary the urogynea nurse all of who said that nothing was available so I made some more phone calls to the medical secretary who told me there had been some problem with the lab. as I was continuing to have some problems still and becoming very nervous [because my brother had bladder cancer several years ago] I just wanted to know the result either way so I telephoned the consultants secretary once again of which she was very helpful and managed to fit me in with the consultant because there had been a cancellation. During my consultation I was horrified to be told that my biopsy had been labelled with another patients details and be filed in that patient's notes and after tracking them down by ways of me being the only patient who had had a biopsy that day so these must be mine. thank god the result is normal. I said it’s disgusting and thank god they are normal albeit I am a little bit frightened to know that my results are based on the fact that I was the only person to have a biopsy that day so the results must be mine.

If mistakes are made with patient’s details on tests then I dread to think of further outcomes.

I am so sorry to read about the confusion experienced with your test results which clearly falls below the standards we expect for our patients. Thankfully this is an extremely rare occurance and, whilst this does not excuse in any way what you experienced, we are in the process of reviewing our processes to identify any further steps we can take to prevent similar incidents happening in the future.
