"Talking therapy service - Missing questionnaire"

About: Somerset NHS Foundation Trust / Adult community mental health

(as a service user),

After my initial appointment I was sent a bundle of reading material and should also have received a questionnaire to complete before my next appointment so that  the answers could be discussed. However, the questionnaire was not included in the pack of material that was sent so that much of the session was taken up  going through the questions. I was frustrated by this as I felt I would have liked more time to consider the responses and then be able to talk about why I had given the answers I had.

I have another appointment in March, I've not received a questionnaire to complete before this so am not sure whether this is an oversight or whether it really is not needed. I don't really want another appointment where so much of the time is taken up with questions that should have been completed before hand.

Dear scorpiuskk39

I'm sorry to hear the promised questionnaire was not sent to you correctly. I commend your interest in these anxiety and depression questionnaires as they greatly assist our assessment and treatment processes. If you have an email address you are happy to share with us, your clinician can send you a link to complete the questionnaires online prior to sessions. Alternatively s/he can give you the form to take away after each session. Please note though that is often useful to do them together for the first couple of sessions to be able to discuss their meaning and use. But as you rightly say, you can still do this even if you have completed them prior to the session. Please do discuss these ideas with your therapist at the next session and mention my message here.
