I found my mother unconscious in her home, called an ambulance and received fantastic service from the crew, the A&E department at Calderdale Hospital, the admissions ward, sadly then things went downhill and my mother was discharged 5 days after admission. No care plan was in place, no aids were initially offered despite her incontinence and a number of other issues. No help was offered whatsoever.
Enter the Rapid Response Team via the Gateway to care. From speaking to her GP she had a visit from a doctor within a couple of hours, and then the whole Rapid Response Team kicked in. We had a visit from Nursing Staff, the Meds Team, Occupational Therapy, you name it they were there for us. A thoroughly breathtaking response, with the exception of the Podiatry service. I would like to pay particular praise to Alice in the Medicine Support Team, no one could have been more caring.
