"Death certificate and end of life care"

My mother passed away in AMU ward after a difficult time last week.

Most of the staff were excellent except the night before she passed away. The nurses were very unprofessional and caused great distress to my mother. We complained to ward manager and no response from her yet.

End of life care was poor in fact it was non existent. Doctors didn’t know doses of medication and no palliative care team visited before she passed away and we had to chase nurses for medications. This is an ABSOLUTE DISGRACE.

The most APPALING thing is the death certificate. We are still waiting for her death certificate as I write this and it is one week since she passed away

It was done by a Junior doctor incorrectly and the bereavement team are completely useless in their approach. They don’t communicate or chase up anything and more over they lie and are very unprofessional. There is absolutely no sense of urgency. I feel the supervision for junior doctors in writing a death certificate is non existent.

We complained to PALS and I am told it takes 9 weeks. That is fine as they have to investigate but no body is taking active responsibility to get the death certificate sorted.

My poor mother is still lying in hospital mortuary. She hated to stay in hospital and we wanted to get her to chapel of rest immediately. This hospital has seriously let us down.

I am not sure how this hospital achieved a Good CQC rating.

I am the Head of PALS, Complaints and Candour at the Trust and I would like to offer my sincere condolences for the loss of your mother at what must be a difficult time for you and your family. I am very sorry you have had cause to post these concerns regarding your mother’s recent experience at the hospital and the experiences you as a family continue to have.

I am aware that you have contacted my team and raised your concerns which are being investigated as a formal complaint. The Coroner’s Office will be contacting you, if they have not already done so, with regards to the death certificate now being completed.

It is very disappointing to read your comments and of your concerns regarding your mother’s end of life care. We take patient concerns very seriously and we can only improve if we know what is going wrong from a patient’s perspective.
