"lack of communication"

About: Lincoln County Hospital / General surgery Lincoln County Hospital / X-ray and Scans

I was first admitted to Lincoln hospital in Nov 2018, with epigastric pain, I had an ultra sound scan and then had an MRI scan, it was discovered I had at least 4 calculi. I was transferred to the SAL where I sat all day only to find I had not been booked in for the procedure.

I was  discharged that week, my notes stated I had an ERCP request was  in endoscopy, later I tried to discover when my appointment would be, after several frustrating phone calls I found I had no appointment booked at all, I contacted PALS, the person I spoke to said they would find out what was happening, but never got back in touch with me.

Unfortunately I had to be re admitted on the a month later with the same problem, I was given day release for christmas, when I returned afterwards I was told no one new anything about me returning to the ward, a nurse got a diary and told me I was due back the next day, a doctor told me to be back for 08. 00hrs as the ERCP started early, I returned to the surgical admissions ward, were I stayed overnight, and then transferred to Greetwell ward, from which I was discharged in late Dec, my discharge notes stated I was due back early Jan 2019 for the ERCP, I made enquires on the Wednesday but once again I had to make several phone calls, only to find out  once again I had not been booked for the procedure. The nursing staff on all the wards were brilliant, but somewhere there is no communication.
