A relative was admitted as an emergency on Friday evening and required a mental health assessment due to attempted self harm. The hospital were going to admit him to Udston for inpatient care but decided to discharge him on Sunday on the understanding that UHW would refer to CRISIS, Older Peoples Mental Health Team and Addictions. I was told by the doctor that the CRISIS team would be in contact on the Monday as it was an urgent referral and the other two teams would also contact him during the week.
This is now Wednesday and there has been no contact from any of the teams discussed. We have contacted LAADS (Addictions) and they have not received any referral for him and have asked that he call and self refer. I have also contacted CRISIS, but they don't have access to the diary at present.
This means that he is still alone with no support and in a vulnerable position.
