"Inappropriate Discharge Measures"

About: University Hospital Monklands / Care of the Elderly (Wards 14,20,22)

Good Afternoon

My sister is currently in ward 21 of Monklands Hospital. We are hoping she will be discharged soon. 

My sister has now had 3 stokes and in addition has diabetes with which she is not hypo aware, and has enlarged arteries where she has episodes of mini strokes, and is likely to experience further episodes as advised by Professor Barber. With her limited mobility she now requires assistance with showering.

In light of her circumstances we feel as her family it is more appropriate for her to have 24hour care and have decided it would be better for her to be discharged to our brothers home where she willl receive this care. As she is only 54 years any carer assistance is a paid service.

2 occupational therapists from ward 21 visited my brothers home yesterday. In light of limited finances it was agreed a carers service of once per week would be paid to help with showering/bathing, and we would manage with bed baths for the remainder of the week. OTs confirmed they would make this referral to the discharge team.

However, today another OT has spoken to my sister and has refused to make this referral to the discharge team and we will need to discuss this with the community social work team. I have contacted my local social work team and they have advised because my sister is still a hospital patient this would be dealt with by the hospital social work department.

I am absolutely disgusted that a vulnerable patient is being treated in this manner and is expected to be discharged from hospital without basic cleansing facilities being in place. We also require her alarm for help to be transferred to our brothers home. We are looking for minimal assistance with basic measures being in place and will try to carry out all other care needs ourselves.

Can someone please help us with the basic measures to be put in place prior to discharge?

Thank you for your response. I have left you a voicemail with contact details. I would also like to add that my sister was discharged from hospital at 2pm on a Friday afternoon with only a partial prescription, no insulin. District nurses were supposed to come out at 4pm that day to provide insulin. They did not arrive until the Saturday. In 3 days my sister had 2 falls. Also, since contracting urinary sepsis in hospital she has now been left with a level of incontinence, which we were not aware of until she got home!

I am surprised at this response. I had a conversation with yourself and the Patient Affairs Department on 22nd November. You advised me to look into private carers arrangements for my sister.

Pamela Grant had provided us with some assistance and this matter is now progressing with community social services.
