"Rose Ward"

I was recently an inpatient here and I can tell you that I am very concerned for both present and future patients.

It had been noted and highlighted to staff about many incomplete/ incorrect paper work and record keeping during my time here. An example was that on my discharge letter it said that I had diabetes and was treated on the ward for this when infact I have never ever been diagnosed with such a condition nore treated for it. Another example was that records regarding food/fluid intake was not noted by staff which meant that the consultant questioned my intake.when I explained to him that a certain member of staff had indeed supervised me eating but not recorded it, the consultant was quick to highlight to other staff  members and it was then recorded correctly.this happened multiple times. however it should not be the responsibilty of the patient to continuously have to ask staff if they have recorded such things.

On my discharge letter there were many discrepencies also and Alot of the information was negeliable. an example was a weight that was recorded when infact I had never been weighed during my stay. Also my discharge date was stated as 2 months PRIOR to my actual admission date-(i couldnt have been discharged 2 months before even being admitted), yet another sign of poor/ incompetent record keeping.

I would be very concerned and believe that ALL staff need some very urgent training and information on the importance of factual, accurate record keeping.(if somebody who doesnt have diabetes and is being treated for it, it may be very dangerous to that patient,thankfully i wasnt treated but again a typical example of poor and dangerous record keeping)
