About: Hmp Elmley Prison

(as the patient),

I was under IC24 care while in prison when released. No medical records given to my new doctor despite requests from them.

It goes to show poor communication and the fact I have numerous medical issues makes it inexcusable.

Dear patient,

I'm sorry to hear you had problems accessing your medical records. We provide those released from prison with a summary of care, and advice to give their GP on how to contact us. We then provide the information directly to the GP upon written request.

If this has not happened if you forward further detail to info@ic24.nhs.uk we will see how we can support you with progressing this.
