My elderly grandmother was admitted to ward 102 following a short admission in AMIA. While I have no issues with her care while on the ward, I was appalled to find out she was discharged to her care home by taxi. Her family were never contacted to pick her up. She was visited every day, if not twice a day, by family members and the ward were contacted regularly for updates. While I appreciate the need for beds, I find this inappropriate as she lacks capacity, is in her 80's and was still unwell on discharge. To put in context, she was readmitted within 3 days. Her family would have taken her home had we been given the option, but we were never contacted.
