"Still no medical records"

About: St Annes Group Practice

I moved from Herne Bay last June and in October registered with a GP in Whitstable.

However St. Anne's surgery still have not forwarded my medical records to my new GP.

This means that they have no information regarding my angiograms which the nurse requires as my cholesterol is being monitored, or details of problems with my left eye which I was referred to Moorfields about.

I know the surgery busy, but when I worked in the NHS, change of address and medical information were a priority.

Please could someone arrange to sendy medical records to my new GP?

Please could you contact the surgery and ask to speak to Helen Sutton or Clare Abbatt so that we can deal with your query as we cannot look in to this without knowing a name, but this is not something that would be delayed
