"No follow up of care"

July 2017 my daughter received excellent care from both the Rapid Response and Home Care Team.

Somehow, after being discharged from their care and into the doctor's care things went wrong.

Apparently her doctor was unaware of her treatment and there was little follow up after her 6 sessions of CBT (which didn't help) except repeat prescriptions.

At the end of the year my daughter decided she was well enough to give up her antidepressants and stopped taking them. She announced that she had a dissociative disorder but didn't want to talk to us about it.

Happy mania followed until everything imploded in April 2018 when she went to the doctor and was put back onto the tablets.

In June she was suicidal.

Her doctor told her to phone the Crisis team for a referral but they, in their wisdom, decided Talk Therapy would suffice.

An assessment was done at the end of August and an appointment letter came through dated 26th September for an appointment on 3rd October.

The Let's Talk team had tried to call her a number of times but couldn't get through.

No wonder as she hung herself on 11th September.

Too little too late.
