"Medical form to be completed by GP"

It was necessary for my daughter (patient of putnoe) to have a health form completed for her to be able to fly to her honeymoon.

This form only came to light from the airlines less than a month prior to her flight which added to the stress she was under leading up to her big day.

Whilst I and my daughter appreciate that this not necessarily a high priority with in the practice as we appreciate that patients come first, we were extremely grateful for the efficiency that this was dealt with.

Both the administrator and the GP who dealt with this were polite, understanding of the situation my daughter suddenly found herself in and did their best to ensure she had the completed form in time, and I'm pleased to say she has been given the all clear to fly with the airline in question.
