I'm glad that you had a generally positive experience in relation to the staff and calling for help. The team will be glad to hear this.

In terms of what you have indicated about making plans for the future - this looks like something we could be doing better at. Without specific information, this one may be difficult to address however we are always striving to be more focussed on what is important to you, and your priorities for discharge and your future. I will share this feedback with the whole team and ensure we make a renewed effort to improve our communication around discharge planning and future care planning.
