"Poor aftercare advice following carcinoma removal"

About: Royal Cornwall Hospital (Treliske) / Dermatology

(as a service user),

Poor aftercare following the removal of a carcinoma on my leg. The leaflet provided when I left merely advised the wound be covered in Vaseline frequently to allow the wound to be soft when stitches were removed. I was urged to keep my leg elevated which I did. My wound wept constantly with lymph liquid and when I attended the surgery after 10 days, I was given a thin wad of lint covered in plastic and told a wet environment was ideal for healing. This dressing could remain for up to 3 days. As I was going on holiday, I dealt with the constant wet dressings and was unable to source anything to cope with the wet wound from Boots. On my arrival in France I was seen by a nurse who immediately sent me to a doctor who subsequently arranged for the wound to be tended every day by a nurse with dressings far more suitable to absorb some of the liquid. The nurse was adamant that the wound should be kept as dry as possible. Removal of the stitches had to be delayed for one day due to the inflammation. No mention in the advice leaflet from Dermatology is made of lymph liquid - only blood. Although they do advise contacting your surgery "if you are unable to change the dressing yourself", I felt I should try to cope as I am perfectly "able". However, my efforts to deal with the wound were leading to a very poor outcome so I did see a nurse after 10 days. Again, the dressings I was given were totally inadequate to deal with the amount of liquid the wound was producing. But this is another issue - with my surgery. I would like to point out however, that the treatment aspect of my experience with RCH dermatology was good.

Dear PNP

Firstly may I apologise for the delay in responding to you on here. I am sorry to learn of the complications you experienced post surgery. Thank you for taking the time to share your feedback.

I have reviewed the leaflet provided to you and it does contain wound after care instructions, however it is unable to cover all possible wound concerns as these are dependant on the individual patient and their medical history. We do always verbally go through the leaflet after surgery and advise our patients to contact us on the telephone number provided or contact their GP if they have any problems.

I am sorry if the information provided caused any confusion and I will ensure that your experience is shared with the rest of our team as we would not want any patient feeling that they were left unsupported.

I hope that the wound is now healing well and I am pleased to read that the treatment aspect of your surgery was good. If you would like to discuss any of your concerns any further, please do not hesitate to contact me via Dermatology reception on 01872 253254.

With kind regards

Claire James, Staff Nurse, Dermatology
