"Wrong information in letter"

I have received a patient experience survey to complete only to discover that my name is completely wrong in the letter to begin with. If this is the level of care taken when sending my letter I wonder who knows who is getting the wrong mail.

Similarly, my sister had her tonsils removed at the earlier this year and the name above her bed was similar to hers but wrong. If this happens to a patient who is unable to indicate the inaccurate details it is worrying to contemplate the out come.
