"Ashley House forgot to contact my GP"

(as a service user),

I believe this place is going downhill fast, if it was ever uphill in the first place. Saw my keyworker a couple of weeks ago. A letter had to be raised and sent to my doctors so that I could get a prescription.

I duly made my appointment with my GP and attended, it quickly became clear, in my view, that yet again AH had failed to do as they had promised.

The letter had not been sent. Indeed, after speaking with AH it appeared in my file unsent / not actioned.

i would like to apologise for this oversight; this issue will be discussed at the team meeting in order to raise vigilance and prevent a re occurrence.
