"Hospital care was great but no discharge planning"

About: Crosshouse Hospital / Urology Renal (Kidney) Medicine & Dialysis University Hospital Ayr

(as a relative),

He spent a week in Ayr hospital then 3 weeks in ward 2D at a crosshouse before being discharged. His care as an inpatient was great, particularly in ward 2D. The nurses were very approachable and caring and his consultant updated the whole family on numerous occasions. Our issue has been his discharge. He was discharged home into a building site (a wetroom was being installed in their only bathroom), he is incontinent and was only given 1 packet of pads, my 75yr old mum is his carer and received no training in his catheter, including swapping the bags at night. The district nurses are not visiting until this Wednesday and we have been forced to purchase pads as they have run out. My mum phoned the gp today to try and get pads and was told they’d need to wait until Wednesday. So to sum up-hospital care great, discharge planning non existent.

Discharge to home with all the appropriate support is so important for so many reasons and I am so sorry that this was not the case. This must be so distressing for you all.

I appreciate that it is probably too late to help improve the current situation for your dad as today is Wednesday and hopefully the DN will sort out things for your dad now. However, it is important that we look at why this happened and plug any holes in the service if and where possible. Thank you so much for contacting us on Care Opinion. It is only by understanding what went wrong that we can fix things.
