"Hospital great but smoking in the grounds ?????"

Yesterday I had an appointment and was seen to very quickly and professionally.
However between the car and hospital I noticed quite a few people either smoking or caping.
There is a big sign which clearly states no smoking on the grounds.
Why don’t the hospital give on the spot fines of say £20.
This would discourage people to adhere clearly to the signs otherwise what was the point.
