(as the patient),

I was brought up to the hospital a few months ago and I understand it was put on my record that I overdosed and was given narcan. This can't be further from the truth and I'm seriously angry as this never happened. Someone is not doing their job right. I think this mistake was made by the ambulance crew. Please contact me to discuss further. This needs to be corrected as this is on my record.
