"lack of care and communication"

About: Medway Maritime Hospital

useless discharging my wifes aunt with out informing relatives. discharging her with out adequate care package and leaving her without any equipment. tablets all over the place. no chair or table which was to be provided no pads no drinking beakers. Milton ward didn't even book transport for dialysis at Canterbury. hospital social worker and OT departments didn't listen to our risks concerns. no communication. im totally fuming totally unacceptable this is 2018 not 1942.
