About: Doncaster Royal Infirmary / Accident and emergency

My mother was admitted to A&E on 12 April 2018 from her Care Home. On my arrival, having travelled 2.1/2hrs in a panic to get to her i was horrified to hear from a consultant Dr that my mother arrived with no notes. I assured the Dr that they had been put on the ambulance by the home as i had previously spoken to them when they telephone to say mum was being admitted to DRI.

As i live some distance away and mum could not give the Dr the information he required due to having cognitive issues i found my mother to be in a anxious state. I would like to know how her notes could of simply disappeared? This did result in confusion re the discharge letter as the home address was correct but the GP information was the previous person. This has since been corrected due to the Home Manager calling the hospital to rectify the mistake.
