Our local surgery has an appointment system: you have to call that morning to get an appointment later in the day. Some can be booked in advance, for example regular appointments for check-ups on medication effects, but it requires some kind of prior arrangement with a particular GP.
Getting an appointment the same day is good as you can tell your work that you won't be in in the morning/afternoon, but can be difficult getting through at busy times (just after phone lines open after a bank holiday weekend for example.)
