"Transport on Discharge from Hospital"

About: University Hospital Monklands / Care of the Elderly (Wards 14,20,22)

(as a relative),

My father who will is in his seventies had a severe stroke in October 2017.  He had a blood clot on the left hand side of his brain affecting his right, his mobility, right arm and leg and also speech all affected and is learning to walk again with 2 aids on one side. 

He spent 3 months in Monklands General Hospital Ward 21. The staff can I say were amazing and so kind to my dad I want to say a big thank you to them.

From there he was discharged in January 2018 which didn't go to plan, there was a mix up in communication and he got hospital transport booked to the door and should have had an ambulance with a stair aid to get him up the 15 stairs, instead of this the hospital transport (who were great) got him to the front door and my brother tried to get him up the stairs,  he then had to crawl on his bottom and left hand side across the top landing to his room and hospital bed waiting, it was a sight that shocked us all and haunts me to this day and would want no family to go through that again. It took him a few days to recover as he was so tired and his right arm which he cannot use was hurt by the event. Please do not let this happen again to any family.

Thank you so much for taking the time to post your feedback on Care Opinion. It is really important for us to listen to relatives and patients' experiences to allow us to improve on our care.

I am delighted that you found the staff in Ward 21 to be amazing and kind and I will ensure that this information is passed on to all the staff. This is the level of care we strive to deliver on a daily basis to all patients in our care.

I am disappointed to hear that your dad's discharge did not go to plan and we need to understand firstly what went wrong in the discharge planning process and determine where we could improve. Please accept my sincere apologies that this happened to your family and to your dad as this is not the experience we would want patients and their families to go through.

In order to understand what went wrong, we would require your dad's details which will allow us to carry out a local review and share learning across University Hospital Monklands.

If you are happy for this to be done, could you please contact Patient Affairs on 01236 713065 and provide us with your dad's details and I will ensure the ward team look into your dad's discharge from hospital.
