"Fast track care from Bradford District Care Trust"

My Mother was in Airedale District hospital, but when it became obvious that she was nearing the end of her life the family wanted to get her home to spend her last days in familiar surroundings.   A referral was done to the Fast Track team in Bradford, and I had a phone call from Martin, a Senior Nurse, who listened to what we wanted and explained everything to me very clearly.   Within a couple of days Martin had put everything in place -  coordinating Carers, District Nurses and GP, and ensuring that all necessary equipment and medication etc. was delivered.   Mum's discharge from hospital was swift, (none of the usual waiting for medication etc. ) and Martin came out to visit us the following day.   I cannot speak highly enough of Martin, the District Nurses and the whole care team for the sensitive way in which they looked after us at such a difficult and stressful time.
