"Lack of communication"

About: The Great Western Hospital / Older people's healthcare

My father has recently been admitted to the hospital. There was no details of his memory loss, previous problems or my details as next of kin. This is despite him being admitted to the hospital last November and again in February. All these should have been on record. Also details do not follow the patient if they are transferred else where for further treatment, nor are they sent to his GP. Surely this could be easily resolved, in the interests of patient safety and good care. Also for the treating medical professional to do their job effectively and efficiently.
