What I liked

The staff were professional and caring, but the dis-functional process let them down.

What could be improved

The communication between hospitals and staff were unsatisfactory. Furthermore once discharged from the hospital, I believe the care and treatment was forgotten and a single responsible person for his well-being was absent and would have created not only a more pleasant experience for the patient, but also a more successful outcome.
