"My father's discharge"

My father, who is in his 90s, was in hospital for 2 weeks following a bad fall at home and complications. During his time on Pemberton ward at Wigan Infirmary the nursing treatment was exemplary and we couldn't have wished for better, however on his discharge home with a "Care Plan" in place it all went wrong. He was discharged early afternoon the carers didn't know he was catheterised he had no bags straps for the bags or night stand. In the evening my niece had to walk a 6 mile round trip to collect 2 bags and strap from the hospital so that his catheter could be sorted for the night. I tried to acquire some more bags from district nurses who were unaware of my dad's predicament. I live 30 miles away and fortunately my niece lives fairly close so the situation was resolved had this not been the case my dad would have been left in limbo. This is totally unacceptable but hopefully a lesson learnt
