New hospital

Mother did receive good nursing care

Communication at discharge: My mother 85yrs old was admitted for the first time ever in her life with heart problems, my brother and I had to ask questions every time when we visited for status of our mother ,the care and treatment was good but communications were poor , my mother is alert and orientated likes her independence but physically due to age has very limited mobility , and I think maybe a little forgetful but for most part knows what is going on . Upon her discharge there were no adequate discharge instructions given there was no call from the so called " liason person " whoever that was I never did find out to contact to discuss her discharge needs or concerns we might when they assessed my mother for discharge or who we could contact , when we asked questions at visiting times we got different answers from the staff on the floor , at discharge she was sat in temporary discharge area and I inquired regarding instructions for a clinic appointment and was told by I assume a nurse behind a desk that instructions would be received in the mail .that was it , when the letter came for the appointment my mother was to go to , it was a week later and would have left her a week with out any medicine, after a phone call a appointment was made for the correct day, her medicines were initially in individual boxes ,and we arrived one afternoon to be told mother was discharged, after which was canceled and mother went to a short stay floor and was discharged after the weekend, with still very little instruction ,but her medicines were in" blistex packages" that was it ,since i do not live here any more I am disappointed, I am a nurse and i know how hard and appreciate how hard everyone works but how things have changed , I had to educate my mother with her medicines and gave her written instructions in a folder as she was unclear as to what to take . I could go on.
