My brother was a patient in Dumfries hospital for over three weeks during his stay and when he was ready for discharge he was assessed by the occupational therapist who recommended a long time care plan. Despite this he was sent home with your team coming into him twice daily. I know what happened with his discharge is not in your remit but I was led to believe that you would be attending him until a long term support would be in place however I am now thinking this is not the case and his visits from you finish soon my question is where do the assessment documents that your team has been filling in daily now go and how do I go about insuring he gets the Care recommend by the occupational therapist and the staff on the ward?
