Bed notes were completely wrong in many ways, leading to inappropriate care and actions by nurses and HCAs. Examples include recording that the patient did not need assistance with eating/drinking (which led to 2 days of little food or drink when family not present to assist), stating that patient able to take own pills/tablets (which led to morning medication not taken until family visit in afternoon), patient recorded as having good hearing (actually deaf in one ear and hearing aid in other, leading to poor communication and assessment of "dementia"), wrongly recorded as residing in a nursing home (thereby setting the wrong criteria for suitability for discharge).
Spent 4 days in MAU where no further samples were taken, and yet it was claimed doctors were reviewing the results on a daily basis. After 2 seizures and further deterioration a different doctor intervened on day 5 (on Ward B6), prescribed new medication and had achieved significant improvements within days.
Despite information recommending patients/families to discuss care, medication, treatment, etc. there was never any one around to do so.
Unsuitable medication prescribed in MAU - a drug that had been prescribed on previous admission in 2016, but had been withdrawn after a few weeks due to side effects. Prescription was queried by family this time, and was immediately stopped, No explanation given, but clearly no one read the more recent patient notes or medication history. A near miss.
