"Care of disabled with other needs"

About: Chorley & South Ribble Hospital

one homeless disabled person admitted from A+E to a ward. Following partial treatment the ward wish to discharge this person back onto the streets without involving LCC adult social services for assessment as is the so called proceedure. Information states that

Chorley Hospital have their own adult social worker,what happened to communication?

This action/attitude makes the saying of a caring service into a national joke.

What caring service?

The excuse made is the need for beds and I am only the messenger.

Is this attitude and disjointed approach only at Chorley.

How long before this person is back in hospital for another spell because no one could be bothered to put a proceedure in place.

The excuse will of course be that the staff are busy or under pressure and dont have time.

I am disgusted at this attitude in this day and age especially on phoning the ward to be informed that everyone is on their break.

Of course everyone is entitled to a break, but all at once?
