"Poor communication and unfair discharge"

I have been left in the dark due to no one sending letters of appointment dates to be told my case is close due to two care co ordinators leaving without letting me know! Haven’t been seen since August 2017 due to the previous one leaving and now my new one has left without even making any appointments with me! This is unfair on people with mental health issues needing the hospital to be on side with your GP to be able to prescribe certain medications that GPs can’t do without the psych team. This has happened twice!
