liason between nurses and doctors

Plans for nursing home transfer, really not helpful for nursing staff to tell the chosen nursing home you'll never cope with my mothers needs.

The discharge planning meeting was attended by a nurse whose knowledge of the patient was minimal. So much that I gained more information from reading the notes at the end of the bed myself. Shame the nurse did not do this. Th excuse that she'd just been off for 2 weeks. Would have been better if this meeting was given a higher priority.

Several times we were told that my mother could not be sat out of bed beceause she needed a specific chair. A referral was apparently made to an OT. No chair arrived until begged for. Eventually the chair was borrowed to try from another patient.

The impression was it was easier to care for her in bed with her toiletting needs. Her quality of life would have been minimal if bed bridden for the rest of her life.

I'm so grateful that the nusing home have arranged a chair which she is now able to sit out in and attend entertainments/exercise classes.He sitting posture has improved .They have treted her urine infection and toiletting is no longer an issue for staff or the patient.

Anything else?

Discharge liason was asked to find what equipment would be available within the area my mother lived. They never did and this would have been useful in helping in descision making.

The impression was that we should just chose a nursing home and that discharge home was not an option.

I would have liked to have had confidence that all staff all the time were providing for optimum attainment. Unfortunately the words "I don't know your Mum that well" were too often said.
