What could be improved
Communication with family
Anything else?
My 89 year old mother who suffers from Alzimers was admitted to Darlington Memorial Hospital on Friday 27th August with a water infection. On Tuesday 7th September she was transferred to Bishop Auckland Hospital.
Both I and her social case worker were under the impression this transfer was for futher physiotherapy but Bishop Auckland Hospital staff say no such request was made by Darlington. The transfer was made by the Patient Transfer bus along with a full load of other patients and her grandaughter was not allowed to travel with her to comfort and reassure her.
Why has a vunerable and confused old woman subjected to a two hour ordeal when there was no need for this transfer and she could have been discharged.
Also her medication was altered by Darlington Hosptial but athough I was the principal carer and administered her medication when at home was not informed about these changes.
My mother died of DVT on Tuesday 14 September after readmission to Darlington Hospital. I do not know whether during the last month of her life this experience contributed to her death.
