"My care has and continues to fall short of Nice"

About: Claybrook Centre

(as a service user),

My experience of the treatment and recovery team in the last 4 years:

1) No opportunity to provide feedback. 

2) Physical health largely ignored. 

3) Never had a full medication review. The only time my prescribed medication is discussed is if I mention it. 

4) Never seen my care plan and don't know that it even exists.
