"Had to return for medications that still weren't ready"

About: Royal Albert Edward Infirmary

(as a carer),

Lack of communication by ward staff, lack of direct patient care. Poorly managed and disorganised discharge procedure and process. No information on follow up or aftercare. On discharge asked to return to the ward at 18: 00 to collect medications, which were not ready.

Attended the ward the following day after discharge and medication was still not available, these were critical medications for a heart complaint and need to be given at the same time every day, therefore medication time was missed.

I felt the Night staff were dismissive of patients.
