"Not happy"

About: University Dental Hospital

I attended my appointment last week at the restorative clinic and I found the nurses,radiographers and student dentists extremely nice and accommodating but a black cloud soon appeared! One of your professors was awful to me! They belittled me, they were rude, ruthless and quite frankly a very nasty person! I hope their students don't adopt this bedside manner! I have had a massive phobia of dentists ever since I was a young child due to a traumatic experience I had, it's taken a lot of courage for me to actually let a dentist talk to me let alone anything else I also suffer from extreme panic attacks around strangers so my partner attended with me as I clam up and can't speak to strangers and the professor was also rude to him and said they want me to speak for myself not my partner! Bear in mind by this point I was hyperventilating and in tears! They said they wouldn't continue with the consult unless I spoke! And then refused to go ahead with a treatment plan! Can somebody please tell me how I'd go about making a formal complaint? The only positive aspect of my visit was that the dental nurses were very nice and comforting especially when they saw how distressed I was.
