"Inadequate levels of care"

Level of care provided is absolutely appauling with no consideration given to personal barriers to accessing services. I personally have severe phone anxiety and yet my crisis plan only has a number to call and letters ask me to call to discuss or be discharged. Told different things by different staff and felt blamed for my worsening situation and non attendance when I was very clear about my difficulties and what i struggle with and was referred for services which we inadequate to my needs. I would go as far as to say I have suffered institutional abuse from this team due to their consistant failure to provide appropriate support and blame culture.
