My mother has been in both King George's and Queen's (same group) within the last month.
At Queen's, no discharge plan was put in place before my mother, who is nearly 90 and has advanced dementia, was sent home.
At KG, the medical staff said she required NHS continuing care, but it has so far taken two weeks and the forms have not even been submitted.
As my mother is being fast-tracked, so has limited life expectancy, that is poor.
The current reason for the delay is that "OT need to speak to me", yet they have no input into the fast track form.
The nursing and ward staff are caring and delightful (although there are too few of them) but the discharge process is appalling, and is causing bed blocking when a woman for whom the hospital can do nothing is being left in limbo because the staff are not faxing over a form.
