"Problems with infusion"

About: University Hospital Wishaw / General Medicine (Wards 5 & 8)

(as the patient),

I attended the medical day unit to receive an infusion. The nursing assistant who was checking my observations failed to check my oxygen saturation and pulse by not applying the probe on my finger at all. Then recorded a false reading on the mews chart. The nurse putting the infusion through did not let the whole amount of the drug run in and binned a quarter of it! The nurse then done exactly the same with the observations, recording false information on the chart. This experience in a whole has frightened me as basic observations were not sufficiently carried out!

Dear Letdownbigtime

Thank you for taking the time to alert me on your recent experience at the medical day unit in Wishaw General. Firstly I would like to apologise that your experience has not met the standard of care that you should expect and receive. Your experience is being taken very seriously. It is our expectation that patients feel safe when in our care and clearly we have failed in this regard.

I'm concerned that the expected process was not followed in line with the organisational protocol. I’d like to reassure you that your concerns are now being investigated fully with the staff concerned.

In the meantime please be assured that all staff will undergo re-training with regards to recording vital signs for all patients.
