"lack of communication, medical history"

About: New Stobhill Hospital / Cardiology University Hospital Monklands / Cardiology/Coronary Care Unit (Ward 18)

(as the patient),

I am concerned having attended hospitals in different health boards. Last year I attended Monklands hospital after having a seizure with a follow up with a consultant in Hairmyres Hospital. Last month I had a cardiology appointment at Stobhill Hospital. The consultant advised there were no details of the seizure or any tests or results on my medical history. My husband and I had to update the consultant with all the details that we knew.

I always presumed your medical records were kept up to date in case you ended up in hospital, all your medical history would be available to whoever treated you.

What happens if someone takes unwell and are unable to pass on a medical history?

Thank you for taking the time to post on Patient Opinion. I am sorry to read about your concerns regarding the sharing of medical history between NHS Greater Glasgow and Clyde and NHS Lanarkshire.

I have discussed your post with our Health Records team. They have advised that the referral to a service should provide the relevant medical history to facilitate the consultation. From your post it is unclear whether your GP referred you to our cardiology department in Stobhill Hospital or if the referral came from NHS Lanarkshire. If the referral came from your GP, then the GP should provide the relevant medical history. If your cardiology appointment at Stobhill Hospital was not related to your treatment at Monklands Hospital, the follow up appointment at Hairmyres Hospital including tests or results relating to this, then the medical history of this may not be made available to the Consultant Cardiologist.

However, following the introduction of the West of Scotland Regional Portal Clinic, clinical staff have two way access to medical records between NHS Lanarkshire and NHS Greater Glasgow and Clyde. This should provide access to clinical information when required.

If your appointment at Stobhill Hospital was related to your care at Monklands Hospital and Hairmyres Hospital then please let me know here on Patient Opinion as we would be keen to look into this further for you in more detail.

My appointment at Stobhill was a follow up cardiac consultant appointment for an ongoing cardiac condition which is followed up on a regular basis by Stobhill. I had been treated for a seizure in Monklands in November 2016 with a follow up consultant appointment at Hairmyres in January 2017. No details of the seizure were available on my records when I attended Stobhill.

Thank you for coming back to me so quickly to me with a bit more information. From your update, I am reading that your seizure was unrelated to your ongoing cardiac condition and as I mentioned above, details of your seizure would not necessarily been shared with the consultant at your cardiology appointment.

However, I accept your wider point about the general sharing of medical history between health boards across Scotland, particularly when someone is acutely ill. Currently, there is not one system across Scotland that would hold all patients medical history. But as above, there are ways that health boards can share this information when relevant.
