(as a staff member posting for a patient/service user),

My staff team always bring my care plan to me as a draft then I can input my opinion and my wishes. I also have the option to view my care plan when required and I can request a one to one with my care co-ordinator. I feel that I am treated on an individual basis and feel that my care is better here than in the NHS.

It is really good to hear a positive experience of care planning. It would be useful to hear further as to what was really beneficial about the experience, so that we can ensure that the process is consistent for everyone. If you want to discuss this further you can speak to your care coordinator, or a member of the patient experience team.
