I am 83 years old with A. A. A and in late stages of C. O. P. D.
I have been fortunate in having one day per week's care, for nearly a year at Ashgate Hospice. Their support and care have helped me greatly at a time when I sorely needed it.
Alas I have reached a stage where I am being transferred back into the care of my local G. P. I have had very little contact with this surgery. I moved from Sheffield last year the doctors at Northern General said it was too dangerous to live alone and I would have to live in a nursing home or with a relative.
I am now living with my daughter in Chesterfield. I am severely disabled and I have very limited mobility. Can you advise me as to where I can access anywhere to point me in the right direction. I will appreciate any information you can offer. My sincere thanks
