"Why do I have to chase my appointments?"

About: Northern General Hospital / Older people's healthcare Royal Hallamshire Hospital / Ear, Nose and Throat

(as the patient),

Why do we have to chase appointments up. ? Used to be when you were leaving the hospital they gave you a new appointment. All that has changed, they say they will send one, but I find myself having to chase them up weeks after I should have had one, when you finally get through to someone, it is still weeks before they fit you in.
