"After care in Kingston-Upon-Thames"

About: Child and adolescent mental health (CAMHS) / Assertive outreach adolescent team

(as the patient),

I was discharged from an assessment hospital in Stevenage and had to re-register with a doctor in Kingston as I had been away from the area for a few years. It took more than a week to come onto the doctor's books. In the meantime, I was contacted by SW HTT and received two home visits by personnel there. They brought my medicine with them, which included that which I needed for a three week holiday. They came on the day and at the time specified and I cannot praise their professionalism more highly. I was very impressed with the service.

Dear Grateful Patient CP,

My name is Claire Reid and I am the Senior Patient Experience Co-ordinator for the Mental Health Trust.

Thank you for posting about your experience with the Home Treatment Team. It is encouraging to hear that while you were waiting to come onto your GP's book, you had support and were able to get medication

I am pleased to see that they were professional and responsive. I will feedback your comments to the team.

Many thanks

Claire
