Brilliant multi disciplinary team care from team looking after my mum in care of the elderly wards Blyth and Kennet. Really good example of careful discharge planning with OT, doctor, family and social worker. To agree a package of care that helped make for a successfuldisvcharge home..Big help from AgeUk too.
